Expressions of Interest Open for Healthy Ageing at Home Grants in South Western Sydney

South Western Sydney Primary Health Network (SWSPHN) is inviting Expressions of Interest from organisations to deliver early intervention programs that support healthy ageing and wellbeing for older people.

The grants focus on helping older adults stay healthy, connected, and independent while living in their own homes, through community-based initiatives tailored to local needs.

Successful projects may receive funding between $100,000 and $200,000 under a total funding pool of $564,430. The contract term will run for up to 12 months and may include both setup and implementation phases.

Priority areas for funding include improving healthy lifestyle behaviours, reducing loneliness and social isolation, better managing chronic disease, and increasing access to existing support services.

The program is looking for innovative, evidence-based initiatives that support older people to remain active, safe, and independent. These may include workshops, education sessions, or community engagement activities targeting priority groups.

An online information session will be held on Monday, 29 June at 2pm via Zoom for interested applicants.
