My Health Record Helps Improve Patient Care and Information Sharing

My Health Record provides a secure digital summary of a patient's important health information. When regularly updated, it gives healthcare providers a more complete view of a patient's medical history and supports safer, more informed clinical decisions.
The system brings together key health information, including immunisation records, hospital discharge summaries, medicines, referrals and test results. This information can be accessed by authorised healthcare providers and patients, including during emergencies.
SWSPHN's Digital Health team helps practices implement My Health Record by providing hands-on support with registration, clinical software integration, staff training and security requirements.
The organisation says these services can improve patient care, streamline practice workflows and support secure sharing of health information across the healthcare system.
