EOI: Multidisciplinary Team Care – Healthy Eating Active Lifestyle (HEAL) Program
Empower those at risk of, or experiencing chronic disease, to better manage their condition through sustainable healthy eating and activity practises. Accredited exercise physiologists and physiotherapists are invited to submit an Expression of Interest (EOI) to receive a grant of up to $42,000 to deliver the Healthy Eating Active Lifestyle (HEAL) Program within South Western Sydney.
