New Video Explains Short-Term Support Pathways for Older Australians

The Support at Home program is designed to help older people stay independent and live at home for longer. Along with ongoing care services, the program includes three short-term pathways that provide extra support when a person’s care needs suddenly change.
The video explains how these pathways work, when they may be useful, and how they can help older people make informed decisions about their aged care options.
It is also designed to support conversations between older people, families, carers, and health professionals about the right care and support at the right time.
Providers are encouraged to share the video with the people they support. It is available in multiple languages, including Auslan.
