New Guidance Helps GPs Refer Patients to End-of-Life Home Care Pathway

The pathway is designed for older people who have been diagnosed with a life expectancy of three months or less. It provides short-term funded support so patients can receive care at home during this stage of life.
The program is intended to work alongside existing state and territory palliative care services.
Support at Home is a wider program that helps older Australians stay in their homes for longer by providing access to services such as nursing care, allied health support, personal care, transport, and respite care. It also includes culturally appropriate support, including services for Aboriginal and Torres Strait Islander health workers.
The Department has also released a new video resource through My Aged Care to help GPs discuss the pathway with patients and families. The material is available in multiple languages.
New GP resources have been published to guide clinicians on eligibility criteria, referral steps, and how to complete required forms. They also include information on care coordination, assessment scoring, and available in-home services.
Separate guidance on the broader Support at Home program outlines service options, urgent referrals, reassessment processes, and support available to help older people remain safely at home.
