Mepolizumab added as a treatment option for some adults with COPD

Mepolizumab, sold under the brand name Nucala, is registered in Australia as an add-on maintenance treatment for adults with uncontrolled COPD, increased blood eosinophils and stable triple inhaler therapy.
What is mepolizumab?
Mepolizumab is a biologic medicine given by injection. It works by targeting eosinophils, a type of white blood cell that can contribute to inflammation.
In some people with COPD, higher eosinophil levels may be associated with repeated exacerbations. Mepolizumab reduces eosinophilic inflammation and may help lower the risk of future flare-ups.
Eosinophil levels can be measured through a full blood count (FBC). PBS eligibility requirements need to be checked before treatment is considered.
Who may be eligible?
The provided PBS criteria include requirements such as:
- A blood eosinophil count of at least 300 cells per microlitre recorded within the previous 12 months.
- At least one severe exacerbation requiring hospitalisation, or at least two moderate exacerbations treated with oral corticosteroids and/or antibiotics during the previous 12 months.
- Use of triple inhaler therapy containing a long-acting muscarinic antagonist (LAMA), long-acting beta2 agonist (LABA) and inhaled corticosteroid (ICS), with appropriate inhaler technique and adherence.
What should patients know?
Nucala is used alongside existing inhaler treatment. It does not replace prescribed inhalers. Patients should continue their current treatment unless advised otherwise by their healthcare professional.
PBS restrictions and prescribing requirements apply. Healthcare professionals should check the current PBS criteria before prescribing.
People with COPD who continue to experience exacerbations can discuss their symptoms and treatment history with their GP or respiratory specialist.
Inhaler technique and regular use of prescribed treatment also remain important parts of COPD management. Healthcare professionals can review whether inhalers are being used correctly and whether treatment is being taken as prescribed.
