Grants Open Soon for Chronic Conditions Prevention and Care

The program aims to help Australians living with, or at risk of, chronic conditions through early detection, prevention, and better management.
Guided by the National Strategic Framework for Chronic Conditions 2026–2035, the program focuses on health promotion, person-centred care, managing multiple conditions, and improving health equity.
Draft FAQs are available to help applicants with eligibility, applications, and assessment criteria. A webinar for prospective applicants will be announced soon. Check the department’s website for updates and registration.
