Tools like My Health Record, electronic prescriptions, Active Script List, and telehealth give staff and doctors quick access to important health information. This improves continuity of care and helps GPs support more patients.
In South Western Sydney, over 90% of eligible aged care homes have accepted telehealth equipment from SWSPHN. Staff have also received training through workshops, webinars, and one-on-one support.
