AMA Flags Concerns and Supports Targeted Fixes in Cardiac Imaging Medicare Review

The AMA said it supports the intent of the reforms, but warned that overly complex item structures and prescriptive rules may create extra administrative burden and impact good clinical practice, particularly in rural, regional and remote areas.
A key concern is that strict eligibility and documentation rules could delay appropriate testing or make it harder for patients to access care when clinically needed.
The AMA supported proposed changes to the “stress echo caveat”, which would allow patients with exertional breathlessness and those with known coronary artery disease and worsening symptoms to access myocardial perfusion studies (MPS) when appropriate, without unnecessary delays.
It also backed removing rigid time limits on repeat testing, including 12- and 24-month restrictions, where clinicians judge that earlier repeat imaging is clinically justified. The AMA said strict time-based rules can delay care and sometimes lead to higher out-of-pocket costs or more invasive procedures.
However, the AMA cautioned against introducing overly detailed documentation and claiming requirements, arguing that they could increase paperwork without improving patient outcomes.
The submission recommended clearer explanatory guidance, practical examples for clinicians, and a stronger focus on monitoring outcomes and appropriateness rather than simply counting service volumes.
The AMA also supported structured GP access to CT coronary angiography (CTCA), provided there are appropriate safeguards in place, and emphasised the importance of keeping myocardial perfusion studies (MPS) available as a viable option.
It further raised concerns that rising cost pressures in nuclear medicine could threaten the viability of private SPECT imaging services, potentially affecting patient access and bulk billing availability.
The AMA said reforms should aim to improve equity and access while avoiding unnecessary administrative complexity in cardiac imaging services.
