New allied health services expand support for Fairfield patients

South Western Sydney Primary Health Network (SWSPHN) has funded five providers through its Multidisciplinary Team Care small grants program. The initiative is designed to improve access to allied health care and support closer teamwork between allied health providers and small or solo general practices in Fairfield.

Although the program focuses on practices in Fairfield, eligible patients from across South Western Sydney can use the services.

Services available

Active Evolution – MoveWell

MoveWell offers supervised group exercise at no cost to eligible people from culturally and linguistically diverse communities.

There are two program options:

  • Type 2 diabetes and obesity or weight management, with up to 12 people in each class
  • Cardiopulmonary health, including chronic obstructive pulmonary disease (COPD), with up to eight people in each class

Four intakes will be held during the year. Each participant receives an assessment from an accredited exercise physiologist and an exercise plan suited to their needs. Sessions take place under supervision in a private clinic gym.

Bloom Healthcare – Fairfield Allied Health Support

Bloom Healthcare provides allied health care for children, adults and older people with different health, disability and wellbeing needs.

Care is delivered in people’s homes, helping those who may have difficulty getting to a clinic. Bilingual practitioners and interpreters are also available.

Healthstin – Connect Care MDT Program

This service provides coordinated allied health care for older people who may be experiencing frailty, reduced mobility, falls risk or difficulty with everyday activities.

The team provides early intervention in the home, with a focus on helping older people remain independent, improve safety and stay connected with their community.

Robment Group – Robment Foot and Lower-Limb Podiatry Service

Eligible Fairfield patients with chronic conditions can receive subsidised podiatry assessments and personalised foot health support.

The service can assist with diabetes-related foot concerns, foot or lower-limb pain, mobility or gait problems, falls risk and pressure-related concerns.

A GP referral pathway is available, with information provided back to the referring GP. Multilingual support is offered in English, Mandarin, Arabic, Italian, Spanish, Portuguese and Hungarian.

Western Sydney University – Psychology Fairfield Diabetes Service

This service provides psychological support for people affected by diabetes and weight-related conditions.

Support is available for concerns including diabetes distress, eating disorders, depression and anxiety.

The service brings together clinical care, research and training as part of a multidisciplinary approach to support long-term health outcomes.
