New Support at Home fact sheets now available

Fact sheets on features of the new Support at Home program are now available.

Support at Home fact sheets are now available to help older people, their families and carers understand key elements of the Support at Home program. 

The new Support at Home program replaces the Home Care Packages Program and Short-Term Restorative Care Programme on 1 November. 

The new fact sheets cover:

  • service classifications and budgets
  • the service list
  • care management
  • Assistive Technology and Home Modifications (AT-HM) scheme
  • End-of-Life Pathway
  • Restorative Care Pathway
  • self management
  • participant contributions
  • hardship assistance (available to those experiencing financial hardship).
