Support your patients with free type 2 diabetes education

These sessions are designed to support patients who are newly diagnosed or already managing type 2 diabetes, and can be delivered either at your practice or a nearby community health centre, depending on what works best for you.

Duration: Two hours

Available times: 9–11am or 2.30–4.30pm

Day: Wednesdays (subject to availability)
What practices need to do

Book a session with South Western Sydney PHN (SWSPHN)

Invite 8 to 25 patients with type 2 diabetes

Choose to host the session at your practice or a local community health centre

Confirm patient attendance the day before the session
Practices can also access support to identify eligible patients.
