New 10-Year Plan Aims to Improve Chronic Disease Care in Australia

The National Strategic Framework for Chronic Conditions, along with a new prevention and care program, sets out a 10-year direction for improving early detection, treatment and ongoing support.

Australian Government said the framework focuses on five key priorities:

  • Encouraging better health and community education

  • Preventing illness and identifying it earlier

  • Delivering coordinated, team-based and person-centred care

  • Improving support for people living with multiple chronic conditions

  • Reducing health gaps for priority populations

The plan shifts away from treating conditions separately and instead looks at related diseases and shared risk factors as part of a broader approach to health reform.

To help deliver these goals, the Government will fund a new Chronic Conditions Prevention and Integrated Care Program. It will provide $109.9 million over three years from 2026–27, followed by ongoing funding of $38.3 million per year from 2029–30.

Funding will be distributed through open and competitive grants to support innovation, collaboration and projects that can show clear results.

Grant applications are expected to open in mid-2026, with funded projects scheduled to begin from December 2026. More details will be released later in 2026.
