Chronic Condition Management Changes: What Allied Health Providers Need to Know

These updates streamline referral processes and increase flexibility for patients accessing allied health services.
Key Changes for Allied Health Providers
While individual MBS allied health items remain unchanged, the referral and care coordination requirements have been simplified:

GPs no longer need to specify the number of allied health services in a referral (though they may still do so).

The allied health provider does not need to be named on the referral.

Team Care Arrangements (TCAs) are no longer required.
These updates are designed to give patients greater control and flexibility in choosing which allied health services best support their ongoing care.
Planning Service Delivery with Patients
Allied health professionals can take several practical steps to support patients referred under a GPCCMP:

Discuss patient priorities and determine how they wish to use their five available MBS allied health sessions.

Coordinate with general practices to clarify which services are included in the management plan and explore opportunities for collaboration such as:

Case conferencing

Group allied health services for people with type 2 diabetes

Encourage patients with private health cover to explore claiming through their insurer, allowing them to use Medicare-funded sessions for other services.

Use the MyGov app or HPOS/MBS Online to confirm a patient’s service usage and MBS eligibility.

Access the My Health Record to review a patient’s care team and health history, ensuring continuity of care and collaboration.
